Kornel D. Balon, Jr., M.D.
Cardiothoracic & Pediatric Anesthesiology
Dr. Balon, Jr. has been practicing for 10 years and previously worked at Central DuPage Hospital in Winfield and MacNeal Hospital in Berwyn. He is also certified in Perioperative Transesophageal Echocardiography. He is Board Certified by American Board of Anesthesiology. He obtained his medical degree from University of Missouri School of Medicine and performed his residency at Yale University School of Medicine.

Russell J. Bodner, M.D.
Specializing in Arthoscopy, Joint Replacement, Sports Medicine, & Trauma
Graduating from the University of Delaware, Dr. Bodner completed his medical school training at the University of California-Los Angeles. Dr. Bodner went on to complete his residency at the University of Pennsylvania and attained a Fellowship in Trauma Surgery in Innsbruck, Austria, under the direction of Dr. Emil Beck. He has also completed a Fellowship in Sports Medicine, training with Dr. J Richard Steadman in Lake Tahoe, California. Dr. Bodner is board certified and a fellow of the A.A.O.S. He is a team physician for the Northern Illinois University Huskies.

Jay D. Burstein, M.D.
Specializing in prostate disorders, prostate cancer and bladder control problems
Dr. Burstein is a Board Certified Urologist specializing in prostate disorders, prostate cancer and bladder control problems (incontinence, interstitial cystitis, and painful voiding syndromes).
He completed his under-graduate work at Drake University graduating Magna Cum Laude and is a member of Phi Beta Kappa. He graduated University of Illinois College of Medicine. His publications include articles on embryology and anatomy published in urology textbooks.
His memberships include being a fellow of the American College of Surgeons and the Society for Urodynamics and Female Urology.

Sajit Bux, M.D.
Dr. Sajit Bux was recently accepted in the Fellow of American College of Surgeons. He was honored as a Pfizer Scholar in Urology and is Board Certified from the American Board of Urology. Because of his research, Dr. Bux's has been involved in publications and CME presentations in DeKalb, IL and other local areas. He is also involved in prostate and bladder cancer research at Northern Illinois University.
Dr. Sajit Bux was among 1,259 Initiates from around the world who became Fellows of the American College of Surgerons (ACS) in November 2009. He is a member of the American Urology Association, the American Association of Clinical Urology, and the Association of American Physicians of Indian Origin.
Dr. Bux's special interests include: minimally invasive treatment of BPH, stone disease, family planning involving vasectomy, erectile dysfunction, male and female incontinence, prostate, bladder and kidney cancers.

Tony Choi, M.D.
Specializing in Shoulder & Elbow
Dr. Choi received his medical degree from Southern Illinois University School of Medicine. He completed his residency at University of Missouri School of Medicine. He completed his shoulder and elbow fellowship at the prestigious University of Florida College of Medicine under Dr. Thomas Wright.

Alan Y. Chow, M.D.
Pediatric & General Ophthalmology
Dr. Alan Chow, an accomplished pediatric ophthalmologist from Glen Ellyn, has joined the medical staff at Hauser-Ross Eye Institute and Surgicenter, Sycamore. Dr. Chow is the inventor of the artificial silicon retina. He completed his undergraduate studies at the University of Chicago and attended medical school at Loyola University Medical Center, where he also completed his ophthalmology residency training. He continued as a fellow in pediatric ophthalmology at the Children's Hospital National Medical Center and in ophthalmic genetics at the Johns Hopkins Wilmer Eye Institute. He has received honors and awards from NASA, the Westinghouse Foundation, the National Science Foundation, the National Institutes of Health and the Tibbets Award at the White House.

Michael Coulson, D.O.
Anesthesiologist
Dr. Coulson is a graduate of Ohio University College of Osteopathic Medicine and Chicago Osteopathic Hospital and Medical Center. He is board certified in anesthesiology and Chief of Anesthesiology at Kishwaukee Community Hospital. Dr. Coulson provides anesthesia care for patients at Midland Surgical Center as well as assisting with Pain Management services.

Jasmeet S. Dhaliwal, M.D.
Cataract & External Disease Corneal & Vision Correction Specialist
Jasmeet Dhaliwal, MD Cornea Specialist Dr. Dhaliwal is a graduate of the University of Illinois Medical School. His comprehensive Ophthalmology residency was completed at the University of Cincinnati and his Cornea and Refractive Surgery fellowship was completed at Henry Ford Hospital & Health System in Michigan. Dr. Dhaliwal is board certified and is an active member of the American Academy of Ophthalmology. He has published several research studies in prestigious ophthalmology journals. With extensive fellowship training, he utilizes the latest technology and techniques in offering vision correction surgery.

Michele T. Glasgow, M.D.
Specializing in Shoulder & Elbow
Dr.Glasgow is a graduate of Johns Hopkins University and Mt. Sinai School of Medicine. Having completed her residency training at Columbia-Presbyterian Medical Center in New York, she went on to complete a Sports Medicine Fellowship at the University of Pennsylvania. In addition, she has completed a Shoulder Fellowship under the direction of Dr. Louis Bigliani at Columbia-Presbyterian Medical Center. Dr. Glasgow is board certified and a fellow of the A.A.O.S. She serves as a team physician for the Northern Illinois University Huskies and has recently completed a clinical rotation in shoulder and elbow surgery at the Mayo Clinic.

Steven G. Glasgow, M.D.
Specializing in Sports Medicine, Knee & Joint Replacement
Dr. Glasgow is a graduate of the University of Pennsylvania, where he attended both undergraduate and medical school. He completed his residency at the University of Pennsylvania as well. Under the guidance of Dr. Joseph Torg, he went on for his Fellowship in Sports Medicine, also at Penn. Dr. Glasgow then spent 5 years as an Assistant Professor of Orthopaedic Surgery at Penn, teaching residents and fellows Sports Medicine. Board certified and a Fellow of the A.A.O.S. & A.O.S.S.M., he is a team physician for Northern Illinois University and a distinguished lecturer for the American Academy of Orthopaedic Surgeons review course in knee injuries.

Raymond Hwang, M.D.
Dr. Hwang received both his Bachelor of Science and his Master of Engineering degrees from the Massachusetts Institute of Technology. He earned his Doctor of Medicine from Harvard Medical School as well as a Master of Business Administration from Harvard Business School. Dr. Hwang interned in the Department of Surgery at Massachusetts General Hospital and then completed his residency in the Harvard Combined Orthopaedic Surgery Residency Program. He served as Chief Resident of the Department of Orthopaedic Surgery at Massachusetts General Hospital. Dr. Hwang then completed a fellowship in Spine Surgery at New England Baptist Hospital in Boston.
Dr. Hwang is a member of the American Academy of Orthopaedic Surgeons and a member of the North American Spine Society. He is Board Eligible for the American Board of Orthopaedic Surgery.

Rajeev K. Jain, M.D.
Dr. Jain joined Sycamore’s MidWest Orthopaedic Institute six months ago following a fellowship in lower extremity reconstruction at the prestigious Scripps Clinic in La Jolla, CA. He is the only orthopedic surgeon in the area who specializes in total joint replacement. He also has a special interest in treating hip and knee arthritis. He graduated from the University of Michigan and earned his medical degree at Michigan State University. He completed his internship and residency at McLaren Regional Medical Center in Michigan. He has authored numerous articles and presentations relating to the hip and knee and is in the process of completing several others.

Darvin J. Javier, M.D.
Anesthesia Associates
Dr. Darvin Javier is a graduate of Chicago Medical School and did additional training at Lutheran General Hospital and Rush University Medical College. Dr. Javier holds Master’s Degrees in Medical Imaging and Medical Physics. He also achieved certification in Fiberoptic Airway Management and is certified by the American Board of Anesthesiology.

Richard Kopolovic, M.D.
Facial Cosmetic Surgery, Breast Surgery, Liposculpture, Skin Cancer Treatment
Dr. Kopolovic has been practicing plastic surgery in the western suburbs of Chicago and Northern Illinois for over 15 years. he received his medical doctorate from the Ohio State University College of Medicine in 1981. He then completed his surgical training at the University of Illinois in 1988. His training continued at Loyola University in plastic surgery from 1988 to 1991. Dr. Kopolovic dedicated time for research during his residency training and has authored publications in peer review journals for which he has received national research awards during his residencies. His private practice consists mostly of cosmetic surgery and reconstructive surgery associated with skin cancer. He is also involved with a part-time residency teaching program.

John F. Lacart, M.D.
Dr. Lacart did his undergraduate work at DePauw University and went to medical school at University of Illinois, Chicago and Rush Medical College. He went on to do his surgical internship at Rush-Presbyterian St. Luke’s. Dr. Lacart did his residency at Northwestern University, Northwestern Memorial Hospital, St. Francis, and Lutheran General Hospital. He did his fellowship at Northwestern University and was Chief resident at both Children’s Memorial Hospital and Northwestern Memorial hospital.

Neil B. Levin, D.P.M.
Dr. Levin is a Board Certified podiatric surgeon. His Undergraduate work was done at Oakland University in Rochester, Michigan in Biology. After graduating from the Illinois College of Podiatric Medicine in 1977, He attended a surgical residency at St. John Macomb Hospital in Mt. Clemens, Michigan.
Dr. Levin has more than 25 years experience in all aspects of foot and ankle care. He has advanced training in wound care, laser surgery and diagnostic ultrasound imaging of the foot and ankle. While always stressing preventative and conservative care whenever appropriate, he prides himself in remaining on the leading edge of new technology for diagnostic and surgical procedures.

Photine Liakos, M.D.
Specializing in Foot & Ankle
Dr. Liakos is a native of DeKalb. She completed both her undergraduate and medical school education at Washington University in St. Louis. Her orthopaedic residency training was at Washington University Medical Center. Under the direction of Dr. Jeffrey Johnson, she went on to complete her Fellowship in Foot and Ankle Surgery, also at Washington University. Board certified and a fellow of the A.A.O.S., she is the team physician for Kishwaukee Community College. Most recently, she completed training in total ankle replacement surgery and is certified in the newest techniques of orthopaedic shock-wave therapy.

Jeremiah Loch, C.R.N.A., Ph.D, D.A.A.P.M.
Pain Management
Jerry is an advanced practice nurse with significant experience as an anesthesia provider and specialized training in the treatment of pain. His certification includes: Advanced Practice Nurse, Anesthesiology Practitioner, Certified in Pain Management (AAPM), Certified in Naturopathic Medicine (ANMA), Certified in Anesthesiology (AANA), Incorporating Advanced Osteopathic Training.

Mark A. Nessim, M.D., J.D.
Anesthesiology
Dr. Nessim went to the University of Illinois College of Medicine (Urbana-Champaign) for his medical schooling and performed his residency at Rush-Presbyterian-St. Lukes Medical Center. He is Board Certified in Anesthesiology.

Robert Swartz, M.D.
Specializing in Hand & Upper Extremity
Dr. Swartz received both his undergraduate and medical degrees from the University of Illinois. He completed his residency in orthopaedic surgery at Columbia-Presbyterian Medical Center in New York City, where he met and worked with Dr. Michele Glasgow. He went on to complete a fellowship in Hand/Upper Extremity Surgery at Harvard University. Dr. Swartz is Board Certified in orthopaedic surgery and has a Certificate of added Qualification in Hand Surgery, issued by the American Board of Orthopaedic Surgery. He is a fellow of the American Academy of Orthopaedic Surgeons. Dr. Swartz spent several years practicing in Connecticut before returning to the Chicago area to practice.

Thomas W. Tilton, D.O.
Cataract & Refractive Surgery Specialist
Dr. Tilton received his medical training at the University of New England, College of Osteopathic Medicine in Biddeford, Maine. He completed his residency in Ophthalmology at Grandview Hospital and Medical Center in Dayton, Ohio. Dr. Tilton, who is board certified, has many years of experience with cataract and refractive surgery and was part of a nationwide Clinical Research Study (CRS) that evaluated safety and efficacy of LASIK surgery. He has the honor of being among the first surgeons in the United States trained to perform ICL surgery. In 2007, Dr. Tilton and his refractive surgery team were presented with the Center for Excellence Award for exceptional surgical outcomes using the Crystalens new technology intraocular lens.

Kenya M. Williams, M.D.
Oculoplastics Surgery Neuro-ophthalmology
Dr. Kenya Williams is an oculoplastics specialist who joined Hauser-Ross Eye Institute in 2007. She recently completed her post-graduate training in oculoplastics, orbit and neuro-ophthalmology at Allegheny General Hospital in Pittsburgh. Dr. Williams is trained to evaluate and treat a variety of conditions affecting the eyelid. These include drooping lids, eyelid tumors and conditions that affect the lacrimal system. She is also trained in orbital and neuro-ophthalmic diseases. This includes treatment of patients who have thyroid eye disease and orbital tumors. She also offers a variety of facial cosmetic procedures and products for patients wishing to refresh their appearance. Dr. Williams completed her residency with the Department of Ophthalmology at State University of New York, Downstate Medical Center, Brooklyn, NY. She completed an internship in internal medicine at Northwestern University Feinstein School of Medicine in Chicago. She received her medical degree from Finch University of Health Sciences/The Chicago Medical School, North Chicago, and received the National Medical Fellowship Award for Academic Excellence in Medicine.

Shane M. York, D.P.M.
Dr. York received his Doctor of Podiatric Medicine from Rosalind Franklin University in Chicago. He went on to complete a comprehensive three year surgical residency specializing in foot and ankle disorders at Yale University Hospital. Dr. York is board qualified by the American Board of Podiatric Surgery and is driven to continue to learn and teach. He has participated in numerous clinical trials and published articles in the Journal of Foot and Ankle Surgery.
